All Forms should be emailed to the below no later than XX / XX / 2019:

Timing is VERY Critical on these forms as we need to turn them in all at once!!

All Players Need the following:

Disney Waiver - Minor (all Players)      Disney Waiver - Adult (Coaches/Admin)        Player Medical Waiver

All Players Out Of State (NOT in Eastern PA)

Florida Players         Maryland Players       PA West Players      Vermont Players        Virginia Players         West Virginia Players

All Players need to submit a copy of their 'Current' Player Pass like the below

Copy player pass on copier in color, scan copy of pass into PDF file. PDF Format ONLY no photos of pass
